East Midlands Primary Care team reminder 

A reminder from East Midlands Primary care team: Your completed QiO Checklist should be complete.

“The revised deadline for submission is Wednesday, 27 August 2025.”

The new cycle of Quality in Optometry (QiO) submissions should now be completed. To ensure that all GOS contracts are compliant across the East Midlands Region, we require contractors to complete and submit the Quality in Optometry (QiO) Contractor Checklist.

The Checklist can be accessed via the Quality in Optometry website. You will require a login and password, which you should already have set up. A new login can also be actioned from this link.

Once completed can you please submit a PDF copy to england.eastmidsoptometry@nhs.net On receipt of your completed PDF Checklist(s), the content will be assessed by East Midlands Primary Care Team (EMPCT) and if there are any discrepancies between the information provided and the information held by EMPCT, you will be contacted for further information or clarification. If your Checklist answers generate an action plan, you will have 28 days to complete those actions.

Once the review process has been completed, a small number of practices will be randomly selected for a contract assurance visit. If you are one of those practices chosen, you will be contacted in advance to arrange a mutually convenient date and time.

Additionally, please note that failure to complete and submit a Checklist in respect of your contract by 27 August 2025 without exceptional circumstances, may automatically result in a contract assurance visit.
